Informácie o tom, že žiadateľ o vstup do ERN nemôže predložiť
iné (ani doplňujúce) dokumenty, ako je uvedené nasledovne. V opačnom prípade
môže byť žiadosť poskytovateľa o vstup do ERN vylúčená z hodnotiaceho procesu:
The applicant shall attach only these three documents
to the application:

Endorsement provided by the national competent
authority. The endorsement must include at least the minimum information
as described in the
endorsement template available on
the website. The endorsement can be completed with other relevant
information that the National Authority might wish to include. The minimum
information shall be included also in English if the endorsement letter is
provided in any other National language.

Letter dully filled and signed by the CEO or Director
of the HCP. In case of Consortiums, only the CEO representing the
Consortium shall sign the letter

Letter dully filled and signed by the clinical
responsible of the HCP. In case of Consortiums , only the clinical
responsible representing the Consortium shall sign the letter
The submitted application
shall not include any other document or statement and in
particular any statement provided by the Board of the Network.
These applications would be
considered ineligible.
